No matter what your situation, it all starts with deciding between three main options. Option one, which is only available at 65, is to delay the start of Medicare coverage and instead continue to be covered by your or your spouse's employer plan. However, if you are not covered by an employer plan, you must choose between option two, Original Medicare (potentially combined with Part D and/or a Medigap/Supplement Plan), or option three, a Medicare Advantage Plan (MA).

Each of these Medicare plan options come with different benefits, costs, and rules; hence, it is important to weigh both options carefully before choosing. Medicare: When & How to Apply

  1. Situation #1. You are already getting retirement benefits from the Social Security Administration or the Railroad Retirement Board when you turn 65: They will send you a form in the mail three months before your birthday to enroll in Medicare Parts A, B & D.
  2. Situation #2: You are not receiving benefits from the Social Security Administration or the Railroad Retirement Board when you turn 65:
    1. You have employer-provided health insurance: You and your employer must complete Form CMS-L564 prior to turning 65. When the employer-provided coverage ceases, you must complete Form CMS-40B at least three months prior to the end of coverage to ensure no lapse in coverage.
    2. You do not have employer-provided health insurance: You must enroll during your Initial Enrollment Period (IEP) which begins 3 months prior to, and ends three months after your birthday.
      1. Enroll on the Social Security website.
      2. Call Social Security at 1-800-772-1213, Monday through Friday, from 7AM to 7PM. TTY users can call 1-800-325-0778.
      3. Apply in person at your local Social Security office.
      4. If you worked for a railroad, you can apply for Medicare through the Railroad Retirement Board at 1-877-772-5772, Monday through Friday, from 9AM to 3:30PM. TTY users can dial 1-312-751-4701.
  3. Situation #3: You do not have drug coverage: You must apply during your IEP or when employer-provided coverage ceases. You are not obligated to buy Part D coverage but may opt out.
